Annual INDIVIDUAL MEMBERSHIPS

Individual memberships are available for $25 per year. 

Benefits include

•  Invitation to all AAI meetings throughout the year,
• Knowledge of current local and national information on asthma related activities and articles
• Knowledge of educational opportunities for health care professionals
• Opportunity to help with AAI's community outreach efforts to educate on asthma education, prevention, and management
• Acknowledgment of your participation in AAI activities

Just fill out the attached 2009 Commitment form and send  a copy with your membership dues to the Asthma Alliance of Indianapolis, 3838 N. Rural St.  Indianapolis, IN 46205
